Please take the form from me, Ying Ying ACL, from 3BN on Monday by 8.30 and do submit the form by Wednesday during activity.

The leader from each group will have to message me and indicate your name, and the choices. Do take note that you can only choose ONE from EACH session , For example , Happy Scrappy and Pop Up Balloon Card. It will be on a first come first serve basis. Please message me by Sunday, 9pm so that I can collate. Thank you!
